Most agencies treat documentation as something assembled at the end of a project when someone requests it. Reconstructed records are visibly reconstructed, and they fail exactly when they matter. These four artifacts are produced continuously, as the work happens.
Decision log
Every material choice, who made it, on what date, against which alternatives, with the reasoning recorded at the time rather than remembered later.
Sourcing record
Where every statistic, image, quotation and claim originated, with the original source retained and reachable for the life of the engagement.
Approval trail
Named approver per asset, per version, with timestamps. Nothing publishes without a recorded sign off from a person, not a role.
Accessibility conformance
WCAG 2.1 AA tested per deliverable and reported with the test evidence, rather than asserted in a proposal and never verified.
Five languages, in house.
Tamil, Telugu, Kannada, Hindi and English handled internally rather than subcontracted. The reason this matters operationally is simultaneity. A campaign that has to launch across states on the same day cannot depend on a translation vendor's queue, and sequential launches create a window where one population sees a message before another does.
Translation is also the wrong word for most of this work. A public health message that translates literally often lands wrong, because the phrasing that carries authority differs by language and by region. What is needed is a version written in each language against the same brief, checked by someone who speaks it daily.
Not a checkbox at the end.
Built in, not retrofitted
Contrast, semantic structure, keyboard navigation and screen reader behaviour designed at the start. Retrofitting accessibility onto a finished build usually costs more than building it correctly and produces a worse result.
Tested with assistive technology
Actual screen reader testing rather than an automated scan. Automated tools catch roughly a third of real accessibility failures and give a passing score on interfaces that are genuinely unusable.
Low bandwidth and low end devices
Public information has to work on a five year old handset on a weak connection. That is a design constraint from the first wireframe, not an optimisation pass.
Reported per deliverable
Conformance evidence supplied with each asset, so the record exists without anyone having to request it later.

Public sector AI use carries a scrutiny burden nothing in the private sector matches. The correct response is not avoidance. It is deployment where the audit trail is complete enough that any output can be traced to its inputs and its approver.
Multilingual production
Parallel language versions produced from one brief, each reviewed by a human speaker before release. Nothing citizen facing publishes on machine output alone.
Citizen query analysis
Helpline and web enquiry volumes analysed to find where public information is failing, which is usually a phrasing problem rather than a coverage problem.
Accessibility assistance
Alt text drafting, plain language versions and reading level checks at volume, with human verification.
Full audit logging
Every generation, prompt, source and approval recorded. If a published sentence is questioned, the trail back to its origin exists.
